Lumen Field Full-Scale Exercise Actor Intake Form

Actors must be 18 years or older. Virtual actor training for this exercise is mandatory and will be coordinated with the Cybersecurity and Infrastructure Security Agency (CISA). Actors who arrive to the exercise under the influence of drugs or alcohol (whether legal or illegal) will be turned away and barred from participating. By registering for this exercise, you agree CISA has permission to use any photography or recording taken during the exercise in future documentation or promotion, and that such shall be without compensation. The CISA Exercise Team will coordinate to make accommodations for participants with access and functional needs, and we ask that you provide information pertinent to your needs when filling out the Actor Intake Form.

Exercise participant safety takes priority over exercise events. All participants involved in the exercise share the same basic responsibility for ensuring a safe environment. In addition, it is important to be aware that an operations-based exercise environment is inherently dangerous. Weapons of any kind will not be allowed (including but not limited to firearms, knives, explosives, chemical irritants, electroshock weapons, and batons). All participants will be screened using a magnetometer and / or a physical 'pat down.' If a prohibited item is detected, the participant will be barred from entering the exercise venue until the prohibited item is removed and secured. Any unsafe behavior will result in immediate removal from the exercise.

On behalf of the Football Northwest LLC d/b/a the Seattle Seahawks, First & Goal Inc. (“FGI”), the Washington State Public Stadium Authority (collectively, the “Lumen Field Parties”), the Cybersecurity and Infrastructure Security Agency (CISA), and all other participating organizations (hereinafter, the “Released Parties”), we thank you for agreeing to participate in our Lumen Field Full-Scale Preparedness Exercise (the “Event”). This Event is scheduled for March 18, 2026. I (hereinafter, “I” or “Participant”) agree to participate in the Event, and acknowledge and agree that participation in the Event is voluntary and involves risks. I hereby acknowledge and agree that participating in this Event may involve risk of bodily harm, illness, injury and/or death (collectively, “Harm”) to the Participant. I acknowledge and agree that the Released Parties (as defined below) are not liable for any Harm or personal property damage incurred during the exercise; and I hereby release, waive, and forever discharge the Lumen Field Parties, CISA, all local, state, and federal agencies participating in the Event, and each of their respective successors, assignees or legal representatives, and any owners, affiliates, officers, members, partners or shareholders (whether direct or indirect), subsidiaries, employees, agents, or any of the foregoing (collectively, the “Released Parties”) from any and all claims, demands, causes of action, damages, losses, or liabilities of any kind, whether known or unknown, arising out of or relating to my participation in the Event, including those arising from the negligence of any Released Party to the fullest extent permitted by law. I agree not to bring any lawsuits nor make any demands nor pursue any complaints against any Released Party, wherever, whenever or however the same may occur, including claims arising out of the negligence of any Released Party, to the fullest extent permitted by law. Nothing in this Participant Waiver is intended to waive, limit, or modify any rights, defenses, immunities, or limitations of liability afforded to (i) the United States, CISA, or any federal employee under applicable federal law, including the Federal Tort Claims Act; or (ii) the Washington State Public Stadium Authority or its officials, employees, or agents under applicable Washington law. I agree to indemnify and save and hold harmless the Released Parties from any causes of action, suits, injuries, damages, losses, costs, expenses, or liabilities arising from or relating in any manner to Participant’s acts or omissions in connection with the Event, whether caused by the negligence of any or all of Released Parties, or otherwise to the extent permitted by law. I authorize, but do not expect, Event staff to obtain emergency medical assistance on Participant’s behalf (if deemed necessary in Event staff’s sole discretion) to any emergency health problems or injuries that Participant may incur while attending the Event; and acknowledge that Participant is solely responsible for any and all medical expenses due to Harm. I agree that I may be photographed, filmed, and/or otherwise recorded (collectively, “Recording”) during the Event, and I hereby consent to such Recording. I hereby grant to the Lumen Field Parties and CISA an irrevocable, royalty-free, fully paid-up worldwide right and license to use the name, image, likeness, voice, biographical information, persona, and any other identifying characteristics of Participant (collectively, the “Publicity Rights”) captured, gathered, or provided in connection with the Event, including any Recording, and to create derivative works thereof, in each case, for any purpose and in any media or forum now existing or hereafter developed. Without limiting the generality of the foregoing, Participant acknowledges and agrees that the license to Publicity Rights granted hereunder may be exploited and/or sublicensed by any of the Released Parties for commercial, marketing, and promotional purposes; and that such use shall be without compensation. I understand that photographs and video usage, as well as sharing information or photos about the exercise online in any form (including but not limited to email, websites, or social media applications) is strictly prohibited before, during, and after the Event. Any prohibited photography and video usage will result in excusal from Event. I acknowledge that all devices capable of recording are prohibited unless authorized, and Event staff reserve the right to collect, delete and/or confiscate unauthorized. I understand that I am responsible for following all guidance and directions provided during the Event’s training and briefings and by the Event’s controllers before, during, and after the Event. I understand that failure to comply with safety measures will result in excusal from Event participation. This Participant Waiver Form shall be governed by the laws of the State of Washington. If any provision is found unenforceable, the remaining provisions shall remain in full force and effect. I have read the above PARTICIPANT WAIVER FORM and fully understand and agree to the terms and conditions on behalf of myself. By checking the “yes” box below, I also certify that I am over the age of 18 and acknowledge that I am giving up legal rights and remedies that may be available to me for the actions and negligence of the Released Parties. This acknowledgment form shall serve as my registration for this Event.

Authority: 6 U.S.C. § 6652c(11) authorizes the collection of this information. Purpose: Your information is requested to volunteer for a CISA exercise. This information is used to gather, train, and engage volunteer actors. Contact information, if provided voluntarily, may be used for follow-up. Routine Uses: The information requested on this form may be shared externally as a “routine use” with participating organizations.
